What makes TPQC unique today?
All services are at no cost to the patient or client.
72% of TPQC staff identify as a member of the community or population we serve.
Low no show rates e.g., mental health 3%
All services can be performed at someone’s home, on-site, or outreach setting- we are completely mobile!
Operate under a syndromic model---- the goal is for less than 15% of patients require follow-up visit from previous visit e.g., designed same day STD results and treatment in 45 minutes or coordinated mental health, social worker, pharmacist, and provider intakes.
Transportation is free to all patients by TPQC staff, Uber, or other public transportation assistance.
Highest positivity rates in Illinois (not including Chicago) STDs--- we are good at finding and getting high-risk populations into services
